Member Contract Master Information Form

Each student must fill out the master information form and contract in order to be an official member of a competition team or the Gael Force Academy. Club contracts can be submitted to Mr. Brown in room J-204.

Donation Form

In order to fund our tournaments and activities, we collect a member donation. Please submit the donation form and check to Mr. Brown or Mr. Taylor by Tuesday, September 25.

Competition & Release

The competition and release form confirms that the student is eligible to attend all competitions in the course of the year. Please leave the competition name blank, as we wil use this form as a template for all the competitions we attend this year.

Volunteer Clearance

The volunteer information form only needs to be submitted once. If parents have filled it out in previous years they will still be on the school's record.

Driver

The driver form establishes an adult as an official volunteer driver for transportation to and from events. In addition, a copy of the driver's license and automobile liability insurance claims must be submitted or emailed to the Student Activities office.
For parents who would like to supervise competitions or build sessions in the robotics room must become fully cleared volunteers, please email Mr.Brown at brownadam@dublinusd.org.
